Anemia
Iron deficiency remains the most common cause of anemia in pregnancy, but evaluation should also consider hemoglobinopathies, macrocytic anemia, hemolysis, and less common marrow or systemic disorders.
Clinical points
- Screening and management should distinguish physiologic dilutional change from clinically important anemia.
- Iron deficiency anemia should be treated; current guidance supports oral iron first-line and IV iron when indicated.
- Severe or atypical anemia warrants broader workup including indices, ferritin, smear, and hemoglobinopathy assessment when appropriate.
- Symptoms, gestational age, and obstetric bleeding risk affect urgency of correction.
General / Transfusion
Hematology consultation is often useful when pregnancy is complicated by significant anemia, thrombocytopenia, bleeding disorders, hemoglobinopathies, or anticoagulation decisions.
Clinical points
- Transfusion decisions in pregnancy should be individualized and driven by symptoms, bleeding, hemodynamics, and the specific disorder.
- Do not assume all hematologic abnormalities are pregnancy-related; some reflect preexisting disease or obstetric emergencies.
- Delivery planning should anticipate hemorrhage, anticoagulation interruption, and neonatal implications where relevant.
Sickle Cell Disease / Thalassemia
Hemoglobinopathies are a major modern focus because current pregnancy care emphasizes universal hemoglobinopathy testing and structured high-risk management when disease is present.
Sickle cell disease
- Pregnancy with sickle cell disease carries higher risk of maternal and perinatal morbidity and merits multidisciplinary care.
- Vaccination review, folic acid supplementation, infection prevention, pain management planning, and serial fetal growth surveillance are important.
- Iron should not be given routinely unless deficiency is confirmed.
Thalassemia
- Pregnancy planning should address transfusion status, iron overload, cardiac function, endocrine complications, and partner screening.
- Management is best individualized with hematology input, especially in transfusion-dependent patients.

Hemophilia Carriers / von Willebrand Disease
Inherited bleeding disorders in pregnancy require anticipatory planning for delivery, neuraxial decisions, postpartum hemorrhage prevention, and neonatal management.

- Pregnant patients with known bleeding disorders or carrier status should have third-trimester factor assessment and delivery planning.
- Potentially affected fetuses and newborns require careful intrapartum management to reduce trauma-related bleeding risk.
- Postpartum hemorrhage risk may extend beyond delivery because factor levels can fall quickly postpartum.

APS remains a key cause of thrombosis and pregnancy morbidity and requires diagnosis grounded in established clinical and laboratory criteria plus individualized anticoagulation planning.
Clinical points
- Think of APS in pregnancy loss, severe placental disease, or maternal thrombosis with suggestive antibody profile.
- Management differs for obstetric APS versus thrombotic APS.
- Care commonly involves low-dose aspirin and heparin-based strategies when criteria are met, with postpartum planning equally important.
Coagulation Disorders & Venous Thromboembolism
Pregnancy and the puerperium are hypercoagulable states, and current management emphasizes risk-stratified prophylaxis, prompt diagnosis, and LMWH-based treatment when VTE occurs.
Clinical points
- LMWH remains the preferred agent for prevention and treatment of most pregnancy-associated VTE scenarios.
- Not all inherited thrombophilias justify the same antepartum or postpartum prophylaxis strategy.
- Delivery timing, neuraxial anesthesia, and postpartum anticoagulation require coordinated planning.
